What it is
TMJ Osteoarthritis describes an age-related change in the TMJ. Cartilage thins, the disc may degenerate, and bone forms osteophytes. Many people have imaging changes without major symptoms. Pain is related to load and control, rather than scans alone.
Care restores smooth opening, builds endurance in jaw and neck support, and guides diet and voice loads.
What happens when it occurs
Capsules stiffen, and muscle effort rises. Gentle mobility, control drills, and endurance exercises can help reduce pain, even when imaging reveals changes.
Common causes and risk factors
- Age and family factors
- Tooth loss or malocclusion
- Bruxism and poor sleep
- Prior joint injury
Who is most affected
Adults in mid-to-late life across Australia. Women present more often.
Typical symptoms
- Aching joint pain
- Morning stiffness and crepitus
- Chewing fatigue and deviation
When to seek care
Seek care when pain limits your ability to eat or speak. Urgent dental review for fever, infection, or trauma.
How we diagnose
History, joint palpation, crepitus timing, range and deviation, and bite and cervical screening. Imaging is used when results will change management.
Management
- Education and load planning
- Opening control and lateral stability
- Soft diet during flares
- Neck and scapular endurance
- Dental collaboration for bite support
- Targeted injections in selected cases
Rehabilitation milestones
Phase 1 weeks 0–2: pain control and control drills.
Phase 2 weeks 2–8: endurance and range, chewing practice.
Phase 3 months 2–4: maintenance and gradual increase in diet variety.
Readiness to progress
- Less morning stiffness
- Opening smoother with reduced crepitus
- Normal meals without flare
Long-term care
- Brief weekly control work
- Two strength sessions for the neck and scapulae
- Dental reviews to maintain bite support
Australia snapshot
- Common in Australian dental and physio practice
- Most improve with conservative care
- Procedures are for selected persistent cases

Imaging change does not predict pain well. Capacity and control do.
Small daily practice protects comfort during meals and conversation.
