What it is
Rib growth plate injuries are overuse irritations of the costochondral or chondrosternal growth plates in adolescents. They present with local tenderness at the front of the chest or along a rib and sharp pain with deep breaths, cough, or rotation.
Care settles pain, restores rib motion and breathing mechanics, and rebuilds trunk and scapular strength, allowing for comfortable participation in school, sports, and everyday tasks.
What happens when it occurs
Repetitive loading from activities such as rowing, swimming, throwing, or coughing can sensitise the growth plate. Guarding reduces rib excursion and rotation. Breathing drills and graded mobility exercises reduce sensitivity, while strength and pacing allow for a return to sport.
Common causes and risk factors
- Rowing, swimming, cricket, and racquet sports with high rotational volume
- Growth spurts with sudden training increases
- Recent respiratory illness with coughing fits
- Poor thoracic mobility and low pulling strength
Who is most affected
Active adolescents in overhead or rotational sports. Both sexes are affected.
Typical symptoms
- Localised chest wall tenderness at a rib junction
- Sharp pain with deep breath, cough, or trunk rotation
- Sleep discomfort when rolling
When to seek care
Immediately for crushing chest pain, breathlessness, or trauma. Otherwise, seek care when pain limits your ability to participate in sports or school.
How we diagnose
History and palpation of the tender junction, thoracic and rib movement testing, and screening to exclude cardiac or pulmonary causes. Imaging is used selectively to rule out fracture or other conditions.
Management
- Relative rest from provoking loads and simple analgesia as advised
- Breathing drills to restore rib expansion and relaxation
- Targeted rib and thoracic mobility
- Scapular and trunk endurance and a gradual return‑to‑sport plan
- Taping for comfort in selected cases
Rehabilitation milestones
Phase 1 week 0–2: pain control, breathing practice, gentle mobility.
Phase 2 weeks 2–6: progressive endurance and rotation strength; interval plan for sport.
Phase 3 weeks 6–10+: sport‑specific drills at full volume when criteria are met.
