What is a Lumbar Spine Stress Fracture?
A lumbar spine stress fracture, also called spondylolysis, is a small crack or defect that develops in one of the vertebrae (spinal bones) of the lower back. It usually occurs where the bone connects the upper and lower parts of the vertebra, known as the pars interarticularis.
This condition is most common in young athletes who perform repeated bending, twisting, or hyperextension movements. It often appears during growth spurts when the spine is more vulnerable to stress and load, usually due to the overloading activity of the spine during periods of high-intensity training.
What Happens in Spondylolysis?
Repetitive strain on the lower back creates microscopic cracks in the bone. If this strain continues without enough rest or recovery, a small fracture can form.
The fracture may cause pain and muscle spasm, but in the early stages, it can heal completely with proper management. If left untreated, it can progress to spondylolisthesis, where the vertebra slips slightly forward.
Common Causes of Lumbar Stress Fractures
- Repeated back extension and rotation movements
- Sports involving arching or twisting, such as cricket, gymnastics, football, and dance
- Rapid growth spurts during adolescence
- Weak core and hip stabilising muscles
- Overtraining without adequate rest
- Poor technique or posture during sport
Who is Most Affected?
- Adolescent athletes aged 10 to 18 years
- Cricket fast bowlers, gymnasts, footballers, dancers, and tennis players
- Males are slightly more than females due to sporting patterns
- Children in high training volumes or multiple sports
Symptoms of Lumbar Stress Fractures
- Localised pain in the lower back, often one-sided
- Pain that increases with arching or twisting
- Stiffness after sport or training
- Pain relief with rest
- Tight hamstrings or hip flexors
- In some cases, referred pain into the buttock or thigh

Diagnosis of Lumbar Stress Fractures
Clinical Assessment
The clinician assesses posture, flexibility, and core strength. Pain with back extension or a positive single-leg hyperextension test may suggest a stress fracture.
Imaging
- X-ray: may show the fracture, especially in chronic cases.
- MRI: detects early bone stress and swelling before a fracture forms.
- CT scan: provides detailed images of the bone for confirmation.
- Bone scan: sometimes used to identify an active stress response.

Treatment Options
Treatment aims to relieve pain, allow healing, and restore full movement and strength. Most young athletes recover fully without surgery.
Non-Surgical Management
- Rest from aggravating sports for several weeks.
- Physiotherapy: core strengthening, flexibility, and posture retraining.
- Chiropractic care: gentle mobilisation and soft tissue work to reduce stiffness.
- Gradual return-to-sport program with movement re-education.
- Bracing may be used for comfort in selected cases.
- Pain relief using heat, rest, and GP-prescribed medication if required.
Surgical Management
Surgery is rarely needed. It may be considered only if pain persists after several months of conservative treatment or if there is progression to vertebral slippage.
