What it is
Capsulitis or synovitis is inflammation of the joint capsule or lining of the TMJ. It follows overload, minor trauma, or a locking episode. Pain is local and sharp, with a biting or wide opening sensation.
Care calms inflammation, restores smooth functioning, and guides a return to a full diet and voice use.
What happens when it occurs
Inflammation increases sensitivity. Guarding restricts translation and creates deviation. Early protection and gradual motion improve comfort and function.

- Recent increase in chewing or speaking load
- Jaw trauma or locking incident
- Bruxism and stress

Adults and adolescents after a flare in jaw use.

History, joint palpation, compression and distraction tests, and opening pattern. We screen bite, cervical contribution, and habits. Imaging only for persistent or atypical cases.
Management
- Relative rest and a soft diet during acute pain
- Cold or heat as tolerated and medication as advised
- Controlled opening and lateral glide drills
- Neck and scapular endurance to offload the joint
- Short course of splint use in selected cases
Rehabilitation milestones
Phase 1 days 0–7: pain control, soft diet, gentle range.
Phase 2 weeks 1–4: expand opening and chewing loads, endurance drills.
Phase 3 weeks 4–8: full diet and voice tasks by criteria.
